    Controlling Lygus plant bugs (Heteroptera: Miridae) with European Peristenus relictus (Hymenoptera: Braconidae) in Canada - risky or not?

    The European Peristenus relictus Loan (syn. P. stygicus) has been considered for biological control of Lygus plant bugs native to Canada. Laboratory and field studies were conducted in the area of origin to evaluate the host specificity of P. relictus. Laboratory choice and no-choice tests demonstrated that P. relictus attacked all non-target species offered (fundamental host range). However, closely related non-target mirids (tribe Mirini) were generally well accepted by P. relictus, while hosts from the tribe Stenodemini were less frequently attacked and less suitable for parasitoid development. To validate the laboratory results, a thorough examination of the parasitoid complex of common mirids in Europe was conducted to determine which non-target species may serve as alternative hosts for P. relictus in a natural situation (ecological host range). When comparing both approaches, the fundamental host range of P. relictus matched its ecological host range. In addition to three Lygus species, the ecological host range of P. relictus in the area of Schleswig-Holstein, northern Germany, contains at least 16 non-target species, including hosts belonging to the subfamilies Mirinae, Phylinae and Bryocorinae. A broad ecological and fundamental host range suggests that P. relictus is a generalist; however, P. relictus was not the primary contributor to parasitism of most non-target hosts studied.     Association between frailty and delirium in older adult patients discharged from hospital.

    BACKGROUND: Delirium and frailty - both potentially reversible geriatric syndromes - are seldom studied together, although they often occur jointly in older patients discharged from hospitals. This study aimed to explore the relationship between delirium and frailty in older adults discharged from hospitals. METHODS: Of the 221 patients aged >65 years, who were invited to participate, only 114 gave their consent to participate in this study. Delirium was assessed using the confusion assessment method, in which patients were classified dichotomously as delirious or nondelirious according to its algorithm. Frailty was assessed using the Edmonton Frailty Scale, which classifies patients dichotomously as frail or nonfrail. In addition to the sociodemographic characteristics, covariates such as scores from the Mini-Mental State Examination, Instrumental Activities of Daily Living scale, and Cumulative Illness Rating Scale for Geriatrics and details regarding polymedication were collected. A multidimensional linear regression model was used for analysis. RESULTS: Almost 20% of participants had delirium (n=22), and 76.3% were classified as frail (n=87); 31.5% of the variance in the delirium score was explained by frailty (R (2)=0.315). Age; polymedication; scores of the Confusion Assessment Method (CAM), instrumental activities of daily living, and Cumulative Illness Rating Scale for Geriatrics; and frailty increased the predictability of the variance of delirium by 32% to 64% (R (2)=0.64). CONCLUSION: Frailty is strongly related to delirium in older patients after discharge from the hospital

    Effect Estimation of an Innovative Nursing Intervention to Improve Delirium among Home-Dwelling Older Adults: A Randomized Controlled Pilot Trial.

    AIMS: Estimating the effect of a nursing intervention in home-dwelling older adults on the occurrence and course of delirium and concomitant cognitive and functional impairment. METHODS: A randomized clinical pilot trial using a before/after design was conducted with older patients discharged from hospital who had a medical prescription to receive home care. A total of 51 patients were randomized into the experimental group (EG) and 52 patients into the control group (CG). Besides usual home care, nursing interventions were offered by a geriatric nurse specialist to the EG at 48 h, 72 h, 7 days, 14 days, and 21 days after discharge. All patients were monitored for symptoms of delirium using the Confusion Assessment Method. Cognitive and functional statuses were measured with the Mini-Mental State Examination and the Katz and Lawton Index. RESULTS: No statistical differences with regard to symptoms of delirium (p = 0.085), cognitive impairment (p = 0.151), and functional status (p = 0.235) were found between the EG and CG at study entry and at 1 month. After adjustment, statistical differences were found in favor of the EG for symptoms of delirium (p = 0.046), cognitive impairment (p = 0.015), and functional status (p = 0.033).     Nursing intervention versus usual care to improve delirium among home-dwelling older adults receiving homecare after hospitalization: feasibility and acceptability of a Randomized Controlled Trail.

    BACKGROUND: Delirium is an acute cognitive impairment among older hospitalized patients. It can persist until discharge and for months after that. Despite proof that evidence-based nursing interventions are effective in preventing delirium in acute hospitals, interventions among home-dwelling older patients is lacking. The aim was to assess feasibility and acceptability of a nursing intervention designed to detect and reduce delirium in older adults after discharge from hospital. METHODS: Randomized clinical pilot trial with a before/after design was used. One hundred and three older adults were recruited in a home healthcare service in French-speaking Switzerland and randomized into an experimental group (EG, n = 51) and a control group (CG, n = 52). The CG received usual homecare. The EG received usual homecare plus five additional nursing interventions at 48 and 72 h and at 7, 14 and 21 days after discharge. These interventions were tailored for detecting and reducing delirium and were conducted by a geriatric clinical nurse (GCN). All patients were monitored at the start of the study (M1) and throughout the month for symptoms of delirium (M2). This was documented in patients' records after usual homecare using the Confusion Assessment Method (CAM). At one month (M2), symptoms of delirium were measured using the CAM, cognitive status was measured using the Mini-Mental State Examination (MMSE), and functional status was measured using Katz and Lawton Index of activities of daily living (ADL/IADL). At the end of the study, participants in the EG and homecare nurses were interviewed about the acceptability of the nursing interventions and the study itself. RESULTS: Feasibility and acceptability indicators reported excellent results. Recruitment, retention, randomization, and other procedures were efficient, although some potentially issues were identified. Participants and nurses considered organizational procedures, data collection, intervention content, the dose-effect of the interventions, and methodology all to be feasible. Duration, patient adherence and fidelity were judged acceptable. Nurses, participants and informal caregivers were satisfied with the relevance and safety of the interventions. CONCLUSIONS: Nursing interventions to detect/improve delirium at home are feasible and acceptable. These results confirm that developing a large-scale randomized controlled trial would be appropriate. TRIAL REGESTRATION: ISRCTN registry no: 16103589 - 19 February 2016

    An autoparasitoid wasp, inferior at resource exploitation, outcompetes primary parasitoids by using competitor females to produce males

Autoparasitoids are intraguild consumers that attack and kill heterospecific and conspecific parasitoids as well as immature stages of hemipteran hosts, such as aphids, whiteflies and soft scales. Field experiments assessing the importance of interspecific competition between autoparasitoids and primary parasitoids, as well as its impact on herbivore suppression, are scarcely found in the ecological literature. 2. Using field data from 40 olive orchards, this study examined the mechanisms that regulate: (i) the interspecific competition between primary parasitoids of the genus Metaphycus and the autoparasitoid Coccophagus lycimnia; and (ii) the density of their shared herbivore host, the soft scale Saissetia oleae. 3. Metaphycus parasitoids used smaller hosts than C. lycimnia, yet did not outcompete C. lycimnia. On the other hand, C. lycimnia preferred to use Metaphycus females as secondary hosts for producing males rather than their own females. This preference might explain why the autoparasitoid negatively affected the density of the primary parasitoids. 4. Parasitism by the autoparasitoid C. lycimnia at the beginning of the season was the sole variable positively related to host mortality throughout the season, showing its greater effect on herbivore suppression. 5.     Potential for classical biological control of the potato bug Closterotomus norwegicus (Hemiptera: Miridae): description, parasitism and host specificity of Peristenus closterotomae sp. n. (Hymenoptera: Braconidae)

    The potato bug, Closterotomus norwegicus (Gmelin) (Hemiptera: Miridae) is an introduced pest of lucerne, white clover and lotus seed crops in New Zealand and a key pest of pistachios in California, USA. Efforts were made to identify potential biological control agents of C. norwegicus in Europe. A total of eight parasitoids, including six primary parasitoids from the genus Peristenus (Hymenoptera: Braconidae) and two hyperparasitoids from the genus Mesochorus (Hymenoptera: Ichneumonidae), were reared from C. norwegicus nymphs collected in various habitats in northern Germany. With a proportion of more than 85% of all C. norwegicus parasitoids, Peristenus closterotomae (Hymenoptera: Braconidae), a new species, was the most dominant parasitoid, whereas other parasitoid species only occurred sporadically. Peristenus closterotomae did not fit in the keys to any described species and is described as new to science. Parasitism caused by P. closterotomae was on average 24% (maximum 77%). To assess the host specificity of parasitoids associated with C. norwegicus, the parasitoid complexes of various Miridae occurring simultaneously with C. norwegicus were studied. Peristenus closterotomae was frequently reared from Calocoris affinis (Herrich-Schaeffer), and a few specimens were reared from Calocoris roseomaculatus (De Geer) and the meadow plant bug, Leptopterna dolobrata (Linnaeus) (all Hemiptera: Miridae). The remaining primary parasitoids associated with C. norwegicus were found to be dominant in hosts other than C. norwegicus.     Une étude a été effectuée dans des vergers commerciaux et non-traités du Québec et de la Nouvelle-Écosse pour déterminer l’abondance et la diversité des parasites de la mineuse marbrée, Phyllonorycter blancardella [Lepidoptera : Gracillariidae]. Au Québec, 29 espèces de parasites ont été retrouvées et elles appartenaient à 7 familles, soit Aphelinidae, Braconidae, Chalcidae, Eulophidae, Ichneumonidae, Pteromalidae et Scelionidae. Les espèces les plus fréquentes étaient : Pholetesor ornigis (67 %), Sympiesis marylandensis (11 %), S. serviceicornis (7 %), Pnigalio maculipes (1,5 %), Tetrasticus spp. (1,2 %). Toutes les autres espèces représentaient moins de 1 % des espèces trouvées. Pholetesor pedias, une espèce braconide relâchée à Frelighsburg, Québec en 1983 n’a pas été détectée en 1984 et 1985. En Nouvelle-Écosse, 19 espèces ont été trouvées et elles appartenaient à 5 familles, soit Braconidae, Chalcidae, Eulophidae, Ichneumonidae et Pteromalidae. Les espèces les plus fréquentes étaient : Pholetesor ornigis (52 %), Pnigalio maculipes (14 %), Sympiesis serviceicornis (12 %), S. marylandensis (9,5 %), Sympiesis spp. (5 %), Horismenus fraternus (1,8 %), Paraleurocerus sp. (1,3 %), Stictopisthus flaviceps (1,1 %); toutes les autres espèces représentaient moins de 1 % des espèces trouvées. Sept et cinq espèces d’hyperparasites ont été retrouvées en Nouvelle-Écosse et au Québec, respectivement. Pholetesor pedias, a braconid released in 1983 at Frelighsburg, Quebec, was not found in the 1984 and 1985 surveys.     Effect of prevention measures on incidence of human listeriosis, France, 1987-1997.

    To assess the impact of preventive measures by the food industry, we analyzed food monitoring data as well as trends in the incidence of listeriosis estimated through three independent sources: the National Reference Center of Listeriosis; a laboratory-based active surveillance network; and two consecutive nationwide surveys of public hospital laboratories. From 1987 to 1997, the incidence of listeriosis decreased by an estimated 68%. A substantial reduction in the proportion of Listeria monocytogenes-contaminated products was observed at the retail level. The temporal relationship between prevention measures by the food industry, reduction in L. monocytogenes-contaminated foodstuffs, and reduction in listeriosis incidence suggests a causal relationship and indicates that a substantial part of the reduction in illness is related to prevention efforts

    Intestinal transplantation in children under FK 506 immunosuppression

    Intestinal transplantation, solitary (n = 3) or in combination with the liver (n = 7), was performed in 10 pediatric patients with intestinal failure. The liver was only replaced if there was liver failure and portal hypertension. Immunosuppression was based on FK 506. Two patients died, one of graft-versus-host disease and one of lymphoproliferative disease. One patient was still in the intensive care unit 1 month posttransplantation due to perioperative complications. The function of the intestinal grafts in the remaining patients is normal. All nutrition and medications including immunosuppression are being administered enterally. This series indicates that small bowel transplantation, alone or in combination with the liver, is feasible in pediatric patients. © 1993

    Comparing the characteristics of snowboarders injured in a terrain park who present to the ski patrol, the emergency department or both

    Ski patrol report forms are a common data source in ski/snowboard research, but it is unclear if those who only present to the emergency department (ED) are systematically different from those who see the ski patrol. To determine the proportion and characteristics of injured snowboarders who bypass the ski patrol before presenting to the ED, three groups of injured snowboarders were compared: presented to the ED only, ski patrol only and ski patrol and ED. Data were collected from ski patrol Accident Report Forms (ARFs), ED medical records and telephone interviews. There were 333 injured snowboarders (ED only: 34, ski patrol only: 107, both: 192). Ability, time of day, snow conditions or drugs/alcohol predicted ED only presentation. Concussions (RRR: 4.66; 95% CI: 1.83, 11.90), sprains/strains (RRR: 4.22; 95% CI: 1.87, 9.49), head/neck (RRR: 2.90; 95% CI: 1.48, 5.78), trunk (RRR: 4.17; 95% CI: 1.92, 9.09) or lower extremity (RRR: 3.65; 95% CI: 1.32, 10.07) injuries were significantly more likely to present to ski patrol only versus ski patrol and ED.
